Appointment, Health Questionnaire, and Results

Student Health Administration System

Students can use the Student Health Administration System through Campus-G to make health check-up appointments, fill out health questionnaire, and view results online.

    If you lose or forget your user ID or password, contact the Information Management Core Gifu University (IMC).

Making an Appointment for Your University Health Checkups

You can make an appointment for health checkups. Print out your appointment slip and bring it, or show the screen of your appointment at the reception area.

Filing Out Your Health Questionnaire

Fill out your health questionnaire by yourself prior to your periodic health check-up.
